A photo of yourself (the carer) from your photos, or take a photo
A payment of £8.00

What type of carer are you? (choose one)
Carer

? You provide support to a spouse, partner, child, parent, relative, friend or neighbour who cannot manage without your help. This includes those receiving any financial support i.e. carers allowance.

Young carer

? A young person under the age of 16 years old providing support to a spouse, partner, child, parent, relative, friend or neighbour who cannot manage without your help. This includes those receiving any financial support i.e. carers allowance.

Professional carer

? You are a professional carer who has trained and is paid to look after people. Paid carers include, home care workers, care agency workers, support & social workers, etc.
